USCIS (United States Citizenship and Immigration Services) Medical Examination
USCIS (United States Citizenship and Immigration Services) may require permanent resident (green card) applicants to obtain a medical examination/immigration physical. Only doctors designated by the USCIS as civil surgeons can complete this required immigration exam and sign the required I-693 form. - What should I bring to the appointment?
- - Any USCIS correspondence such as letters, upcoming appointment dates, etc.
- - Any immunization records such as those from childhood, school, or employee health
- - A copy of your medical records such as medications, medical conditions including any records of treatments of tuberculosis or latent, syphilis, gonorrhea if applicable
- - Any copies of previous civil surgeon medical examinations if applicable
- - Your passport or another form of government identification such as a, driver's license, birth certificate, etc.
- - An interpreter if you cannot speak and understand English
- Should I complete the I-693 before the visit?
- You may fill out any information that pertains to you and/or your interpreter or preparer. Do not sign the form prior to your visit. You must sign the form in the presence of the civil surgeon. The staff will provide you a printed copy of the I-693 at the time of your visit if you do not bring your own.
